Payment is expected at the time of your visit.  If you have dental insurance we will provide you with a completed claim form to submit to your insurance carrier.  You are the insurance carrier's customer and experience has proven that you will receive your reimbursement in a timely manner vs. the insurance sending it to a third party.

We require 24 hour notice to cancel an appointment as we have reserved this time for you and are unable to schedule another patient on short notice.  Should you miss two recall visits or New Patient appointments you will no longer be considered a patient of this practice.

Should your treatment plan be extensive we will assist you with financial options.  Our value system allows us to provide you options for treatment.  We will always make recommendations for your dental health.  If these recommendations are beyond your means, let us know and we will work with you to schedule treatment to be more affordable.
